 Miller-Dwan Foundation Grant Proposals - 4 Easy Steps

1.  Review available funds below to determine eligibility, and contact the
     Miller-Dwan Foundation office to discuss your request

2.  Review Application Guidelines and Proposal Format and write
     your proposal based on these criteria

3.  Complete Grant Application Cover Sheet

4.  Mail your completed proposal to:

Miller-Dwan Foundation Unrestricted Fund*
 

*Funding from the Miller-Dwan Foundation
Unrestricted Fund suspended through 2012.

Eligible entities include:
Essentia Health Duluth (formerly Miller-Dwan Medical Center)
Community Nonprofits

Programs and services must fit within the grant-making interests and historical specialty areas of the Foundation. Projects may fall along a spectrum from prevention activities, to assistance within the medical community, to community-based programs.

Van Gorden Fund - New Initiatives Outreach

Eligible entities: 501(c) 3 non-profits in our community who are working to meet the needs of people with disabilities.

The Van Gorden New Initiatives Outreach Fund was established to support and promote new and innovative projects of rehabilitation services, research and education and to foster creativity and excellence in rehabilitation services programming.  Programs and projects must show proof of established rehabilitation goals and further the work of Polinsky and Essentia Health / Miller-Dwan Rehabilitation Services.

Van Gorden Fund - New Initiatives Polinsky

Eligible entities: Essentia Health / Miller-Dwan Rehabilitation Services and Polinsky Medical Rehabilitation Center.

The Fund was established to support and promote new and innovative projects of rehabilitation services, research, and education at Polinsky and to foster creativity and excellence in rehabilitation services programming.

Van Gorden Fund - Sara M. Young Education Fund

Eligible entities: Polinsky and Essentia Health / Miller-Dwan Rehabilitation Services for projects that provide effective and innovative education programming to rehabilitation patients, family members and/or practitioners in the areas as follows: library resources, rehabilitation programming, workshops and seminars, and educational materials.

The Sara M. Young Fund was established in honor of Sara M. Young, a longtime staff member of Polinsky and supporter of rehabilitation and services to people with disabilities.

Albert G. Gonska Fund

Eligible entities: Essentia Health / Miller-Dwan Rehabilitation Services and Polinsky Medical Rehabilitation Center.

The Albert G. Gonska Fund was established with a gift from the will of Albert Gonska. Mr. Gonska was a long-time social worker who dedicated his life to the welfare of children and their rehabilitation needs. The Gonska Fund is therefore dedicated to high-quality, innovative rehabilitation services and programming for children.

DeForest E. Voss Fund

Eligible entities: Essentia Health Duluth and other 501 (c) 3 non-profits to further research efforts focusing on kidney disease and dialysis.

The DeForest E. Voss Fund was established with a gift from the will of DeForest Voss.  Mr. Voss felt strongly about opportunities for improved health for patients with kidney disease and those receiving dialysis care.

Dorothy Ribenack Fund

Eligible entities: Polinsky Medical Rehabilitation Center and the Essentia Health Fitness and Therapy Center.

The Dorothy Ribenack Fund was established with a gift from the will of Dorothy Ribenack.  This fund supports and promotes new and innovative rehabilitation service projects that foster creativity and excellence including research, education and facility improvements.

Abilities First Fund

Eligible entities include:
Individuals with physical disabilities and limited financial resources who reside in our geographic service area.  Eligible applicants are restricted to those individuals who have a primary diagnosis of spinal cord injury, stroke, brain injury, congenital disability, or other documented neuromuscular or neurological trauma or disorder.

The Abilities First Fund purchases physical rehabilitation equipment and aids that allow those with physical limitations to live life more fully and integrate in the community, workplace, or school.

Designated Funds

Eligible entities include:
Internal departments focusing on the historical specialty areas of Essentia Health Duluth (formerly Miller-Dwan) and Polinsky; and Solvay Hospice House

Designated Funds are used for specific health areas and have been created through the generosity of numerous community individuals and employees.  Funds are used for purchases or projects that assist in patient care and comfort, including new equipment, environmental improvements, patient and family education and staff education.
